Annual Compliance Reporting
I can’t open my form in Portable Document Format (PDF). What can I do?
An Annual Compliance Report (ACR) is required for all nuclear substances, radiation devices, and Class II equipment and facilities licences. Each licence has a licence condition specifying the due date for ACR submission.
ACR Forms submission
ACR form can be downloaded and completed either as a hard copy or electronically. Once completed, ACR forms can be submitted to the CNSC in one of four ways:
- attached to an email message and sent to email@example.com
- by fax at 613-995-5086
- by letter mail
- by courier

The Annual Compliance Report (ACR) is a requirement in licences for all nuclear substances, radiation devices, Class II prescribed equipment, licensed activities and facilities, in accordance with subsection 24(5) of the Nuclear Safety and Control Act (NSCA). Collection of the information is used for monitoring licensee compliance with regulatory requirements and licence conditions, and also for studying safety-related data trends in the nuclear industry, which are disseminated to the public in Nuclear Substances in Canada: A Safety Performance Report.
The personal information on this form is required as part of the regulatory framework used to regulate the industry through licensing, compliance and enforcement. All information collected is protected by the provisions of the Privacy Act. If the noted information is not provided, the Canadian Nuclear Safety Commission (CNSC) will not be able to collect the necessary information for determining the licensee's compliance with licence conditions and the regulations established under the NSCA. If the licensee does not submit the ACR, the licensee will be deemed non-compliant with their licensing conditions; the CNSC will take enforcement actions as a result. The information compiled will be used and disclosed only as part of the regulatory approach for compliance and enforcement and is described in the CNSC Info Source Chapter.
Select the applicable form from the list below. ACR forms are grouped by category and customized for the activity authorized by the licence. The licensed activity name and number (e.g. "industrial radiography (812)"), can be found on each licence.

These forms may be updated from time-to-time, so please ensure that you always use the most current version available in the online system or as a downloadable form on the Web site.
The information requested for specified forms may be included as a separate spreadsheet. Please ensure your spreadsheet uses the same headings as the referenced table and contains all required information.
